Exciting opportunity to join our medicine optimisation team based at our units within Torbay Hospital site.
You will be based within our inpatient team and will be an integral part of the Multidisciplinary Team and help to embed medicine optimisation pharmacy services. Your skills and experience will be pivotal in supporting this by ensuring pharmaceutical care and medicines optimisation are fully implemented to the highest standards. Main duties of the job
Core Responsibilities

  • Medicines Reconciliation: Conduct medicines reconciliation in line with NICE guidance and local SOPs, ensuring accurate and safe transfer of medication information.
  • Stock Management: Oversee ordering, monitoring, storage, and destruction of stock and named patient medication.
  • Drug Chart Management: Carry out drug chart rewrites to reduce transcription errors and improve safety.
  • Patient Counselling: Provide regular face-to-face counselling to patients about their medicines.
  • Liaison with Pharmacies: Ensure timely and accurate supply of medication by coordinating with external pharmacies.
  • Audit and Compliance:
  • Conduct quarterly Controlled Drug (CD) audits and storage audits on wards and in Community Mental Health Teams.
  • Regularly check medication charts to inform performance indicators and ensure compliance with service standards.

  • Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC).
  • NVQ Level 3 in Pharmacy Services and/or BTEC National Diploma (or equivalent).
  • Medicine management/optimisation training.
Desirable
  • Accredited Checking Technician (ACT) qualification.
  • Completion of a Medicines Management or Clinical Diploma (e.g. Level 4).
  • Additional training in mental health pharmacy, clinical prioritisation, or patient counselling.
