Are you an enthusiastic and forward-thinking Pharmacy Technician looking to take the next step in your career? The Great Western Hospitals NHS Foundation Trust is seeking a motivated Medicines Optimisation Pharmacy Technician to join our dynamic and supportive clinical pharmacy team.
In this role, you will play a key part in ensuring safe, effective, and efficient use of medicines across our hospital. Working closely with pharmacists, nursing staff, prescribers and wider clinical teams, you will support high-quality patient care by delivering accurate medicines reconciliation, improving medicines safety, and helping patients get the most from their medicines.
The pharmacy team are forward thinking and continuously strive to improve our ways of working, putting medicines optimisation at the heart of our clinical services. We put the patient at the center of our care and aim to enhance every patient's experience.

Supporting safe and effective discharge through processing discharges at ward level and counselling patients about their medicines.
Participating in ward-based medicines optimisation initiatives to reduce medication errors and improve outcomes.
Helping implement best practice, formularies, and medicines policies across the Trust.
Supporting medicines supply, stock management, and minimising waste.
Using digital systems to improve the flow of medicines information and support clinical decision making.
Acting as a role model for high-quality, patient-centred medicines management.

A registered Pharmacy Technician with the General Pharmaceutical Council (GPhC).
Experienced in hospital or primary care pharmacy (though we welcome motivated candidates looking to broaden their experience).
