Slough OPMH has an exciting opportunity for a Mental Health Nurse who wishes to join our dynamic Memory Clinic team. Working alongside a very experienced and efficient MDT is just one of the attractions this opportunity will provide to you.



You will provide nursing support to the memory clinic MDT by undertaking initial assessments and reviews of patients referred to the memory clinic either at the clinic or in the patient's own home/ care home.



You must have knowledge of older people with mental health problems and/or cognitive difficulties. You will be required to travel within the community for the performance of your duties.

Combined with being an 'Outstanding', well-led, organisation, and having an 'Outstanding' Older Adults Community Mental Health Team in our latest CQC inspection. The post holder would therefore be expected to work towards helping the service maintain these accolades of high standards



To work as a Memory Clinic Nurse in the Slough Memory Clinic.



To undertake initial assessments and reviews of patients referred to the memory clinic, including risk assessments, completing assessment scales and associated social care needs.



To undertake home visits /telephone/virtual appointments to monitor cognition, BPSD and medication.



Liaise directly with GPs to ensure appropriate handover of care after review



Provide carer with support and information on medication & its management, educating both patients & carers to understand the illness & its prognosis.



Contribute to the overall functioning of the Memory Clinic.
