Job Description

What you'll be responsible for:

Act as a specialist practitioner, working with a caseload of people referred to the mental health and dementia service. Be responsible for the assessment, planning, implementation, and evaluation of care Adhere to all Trust policies, procedures, and guidelines To work collaboratively with other services to advise and ensure proactive support to people with dementia/mental health in the Trust. Providing frontline support for patients and staff Experience Strong organisational skills and the ability to analyse situations and be able to make the right decisions. Able to empathise, be supportive and sensitive to the needs of others as well as being able to cope with distressing circumstances.



Ability to cope in a pressurised environment and apply practical problem-solving skills in everyday situations. Competent IT skills and the ability to navigate around various systems and software packages (such as outlook, databases, MS office and the internet) Ability to work effectively in a team, work on own initiative, work unsupervised and manage own workload. Manage difficult situations with service users that have complex mental health conditions and dementia. Promote effective liaison with all statutory and voluntary agencies concerned with dementia and mental health.



Working knowledge of the complex needs of people with mental health and dementia. Our current service working hours are Monday-Friday, 8am-4pm.
