Trinity Health Group Primary Care Network (PCN) is a collaboration of GP practices across Wakefield, serving a population of approximately 55,000 patients. As part of our commitment to improving the quality of patient care, reducing health inequalities, and supporting the NHS Long Term Plan, we are expanding our team to further develop integrated, person-centred services.
We are seeking a
Registered Mental Health Nurse
to join our established and supportive Mental Health team. You will provide high-quality mental health nursing care, offering advice, support, and signposting to patients experiencing poor mental health. Working collaboratively with GPs and the wider primary care team, you will play a vital role in strengthening community resilience and delivering coordinated care across our four member practices.
Trinity PCN values the essential role of Mental Health Nurses as part of a wider multi-disciplinary team that includes Care Coordinators, Health and Wellbeing Coaches, Social Prescribing Link Workers, Paramedics, Pharmacists, Pharmacy Technicians, and others across the voluntary, community, and statutory sectors, including the ICB.

Having achieved the independent prescribing qualification is desirable but not essential.
The post will evolve as we develop our services but the postholder will be required to undertake the following duties:

They utilize their clinical skills to promote mental well-being, support recovery, and maximize health outcomes for patients with mental health issues. Relevant Mental Health clinical experience.
Detailed knowledge and appropriate understanding of assessing patients with acute or chronic mental health conditions, procedures and treatment plans. This includes signposting and referrals to appropriate internal and external organisations.
Experience of working within a Multi-Disciplinary Team
Experience of documenting and record keeping of clinical information

Qualifications
Essential
Registered Mental Health Nurse Diploma in Mental Health Nursing or Degree/BSc in Mental Health Nursing.
Evidence of continuing professional development within the last 12 months
