The Proactive Care Team is a new team that aims to develop and deliver a community based integrated model of healthcare for individuals with the most complex health needs. The team will work in the three Community Partnership areas of the Central Locality in Bradford.
Using a Lead Provider model, BDCFT will work in collaboration with key partners including Bradford Teaching Hospitals NHS Foundation Trust (BTHFT) as well as the GP Federations and Voluntary Sector partners to create a proactive culturally competent and aware Multi-Disciplinary Team (MDT). Team members will work to identify those with highest need and deliver a blend of preventative and recovery focussed interventions in the community, to support a reduction of premature mortality and help deliver improvements for those who are frail and those close to their end of life.
The overall aim of the Proactive will be to help reduce health inequalities for those with the most complex needs and support a reduction in Accident and Emergency Department (AED) attendances and unplanned admissions whilst also improving access to other existing community-based provision to help sustain good health outcomes.
To develop and maintain the mental health provision within the Proactive Care Team.
To work as part of a specialist multi-disciplinary health team.
To provide holistic mental health assessments, effective interventions, signposting and liaison with relevant agencies.
