Are you an experienced mental health practitioner passionate about making a real difference in people's lives? PCMHT 3 is a front-door service for the Downham, Bellingham, and Catford areas, providing timely assessments, short-term evidence-based psychosocial interventions, and support to adults with a wide range of mental health needs.
As part of our Primary Care Mental Health Team, you'll work closely with a multi-disciplinary team and partner agencies, including Primary Care, Secondary Care, and MIND, to ensure patients receive the right care at the right time. You'll have the opportunity to undertake triage and trusted assessments, deliver targeted interventions using evidence-based approaches, support families and carers, and contribute to the development of seamless care pathways across the community.
This role offers the chance to work directly within local communities, building strong relationships with GP practices, voluntary organizations, and other services, helping patients stay well, engaged, and supported in their everyday lives. You'll be part of a dynamic team dedicated to high-quality, person-centred care and continuous service improvement.
What you can add:
Manage referrals, providing timely screening, triage, and assessments for adults with a wide range of mental health disorders.
Deliver evidence-based psychosocial interventions for a designated caseload, supporting patients and carers in the community.
Collaborate with MDT and partner agencies to promote engagement, independence, and effective discharge planning.
Participate in MDT governance, clinical meetings, and quality improvement initiatives to enhance mental and physical health outcomes for service users.
Maintain awareness and adherence to Mental Health Law, Care Act, Mental Capacity Act, and other relevant legislation.
Contribute to reflective practice, supervision, and leadership of junior staff.
What you'll need:
Professional qualification in nursing, social work, or occupational therapy with current HCPC/NMC registration.
Post-qualification training, ongoing professional development, and mentorship experience.
Clinical experience working with adults with serious or ongoing mental health problems in community settings.
Skilled in assessment, risk management, safeguarding, and delivering a range of interventions.
Experience working collaboratively in multi-disciplinary teams.
Desirable:
Full driving licence and access to a vehicle.
Knowledge of psychopathology and psychological models (e.g., CBT, DBT, MBT).
