We are currently seeking a skilled and motivated Mental Health Practitioner to join our multidisciplinary team and contribute to the delivery of high-quality, personal-centred care.
What you can add:
Referral and Assessment: Support timely screening, triage, and assessment of referrals for individuals with a range of mental health needs.
Interventions and Care Planning: Deliver evidence-based psycho-social interventions, develop care plans with patients and carers, and provide advice on medication and social prescribing.
Collaboration and Risk Management: Work with the MDT and partner agencies to manage risk, support safe discharge or referral, and ensure clear communication across services.
Professional Engagement: Build links with Primary Care, attend clinical and governance meetings, and stay informed on relevant mental health legislation.
What you'll need:
Relevant registration
Previous NHS experience
The creativity to solve problem and tackle obstacles in new ways
Experience working with children and young people with learning disabilities
Therapeutic interventions with young people with learning disabilities
