Modality Partnership Nottingham Division has a full time vacancy for a skilled and motivated Mental Health Practitioner to join our multi-disciplinary team. The successful candidate will be working across Wellspring Surgery and Windmill Practice within Modality Partnership Nottingham.
This practice-based role enhances timely, holistic mental health care for our diverse inner-city population through a 'first contact mental health worker' model embedded within primary care. You will be a first contact mental health worker embedded in general practice, delivering holistic care through triage, assessment, brief interventions, and reviews. You will work autonomously while collaborating with GPs, PCN mental health colleagues and local services, improving access to mental health support for our diverse inner-city population of 22,000 patients. Main duties of the job
This is a hands-on patient facing role; we are seeking individuals to join our team who are caring, compassionate and confident. Applications are welcome from candidates with a background in mental health nursing, social work, allied health professions, or other relevant fields, with a proven ability to provide safe, effective, and person-centred mental health care.

Key Responsibilities:
Work autonomously to manage a caseload of patients, undertaking comprehensive assessment of physical, social, and psychological needs to develop personalised, holistic healthcare plans.
Create and communicate clear management plans in collaboration with patients, empowering them in their care.
Deliver non-judgemental care, building therapeutic relationships while maintaining professional boundaries.
Undertake triage, initial assessments, brief interventions, and reviews for patients accessing mental health support through the practice triage system.
Be professionally accountable for patient care, providing direct psycho-social therapeutic interventions or signposting to appropriate services as needed.
Provide clinical advice and specialist opinion to GPs and other members of the practice team, both in-person and remotely where appropriate.
Play an active role in the assessment, planning, implementation, and evaluation of patient care, applying safeguarding awareness and management on a case-by-case basis.
Liaise effectively across practice-based care, the PCN Mental Health Nurse, and local secondary mental health services to ensure integrated and seamless patient care.
Contribute navigation and networking expertise to improve access to a broad range of community-based mental health services and support.
Support the development of patient-facing mental health information and support services, including the creation of group-based services and innovative hybrid or online service options in collaboration with the Modality Partnership, and incorporating remote delivery as appropriate.
Complete relevant health checks related to mental health and contribute to continuous quality improvement initiatives within the practices.

Current professional registration with NMC, HCPC or other relevant UK professional body
Experience:
Experience in the assessment and management of people presenting with mental health needs
Proven ability to develop holistic management plans for patients
Experience delivering psycho-social interventions in mental health
Experience working collaboratively within multi-disciplinary teams
Knowledge and understanding of safeguarding practices and processes
Have experience in acute mental health assessment and management.
Skills:
