Our service covers the urban and rural areas of West Somerset, from the Quantock Hills to Exmoor. These positions are dayshifts and are usually 8am-4pm, 12pm-8pm or 8am-9pm. We offer flexible working solutions where possible to ensure a work-life balance and part-time hours will be considered.
You'll need to be a car driver with your own vehicle to visit patients. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation.

Main duties, tasks & skills required

As a community RMN, you'll provide care to patients in their homes, managing a range of complex health issues and collaborating with various other disciplines. Triage referrals, completing initial assessment to understand what is needed and plan what the involvement from IDS will be. Referrals on to other services as appropriate
You'll also work closely with the OPCMHT, D2A and other services, focused on preventing acute admission. You'll develop a broad range of nursing skills, in support the management of behaviours that cause concerns.
You'll need to make autonomous clinical decisions and create holistic care plans, with the support of your colleagues and Team manager

Job description

The post holder may be required to work flexibly to meet the needs of the service.
To work as a member of the Intensive Dementia Support Service, providing an alternative to hospital admission for people with dementia.
You will take charge of a shift to be personally responsible and professionally accountable for a caseload within the community working with people with severe organic mental illness who have multiple and complex needs.
To provide a high standard of care and treatment for patients and work with other members of the team and other agencies to best meet the patients health and social needs.
To work with a team caseload with shared responsibilities for patients whilst under the care and treatment of IDS

Degree/Diploma Occupation Therapy, Mental Health Nursing or Social work
State Registration of HCPC or NMC registration
Membership of professional body plus short specialist course, experience to post-graduate equivalent diploma level.
