Main duties, tasks & skills required

Deliver high-quality, recovery-oriented mental health care through assessment, planning, and evidence-based interventions.
Manage a caseload under supervision, promoting a strengths-based approach to recovery.
Conduct risk assessments and create person-centred Recovery and Risk Management Plans.
Liaise and collaborate with families, carers, partner agencies, and voluntary sector providers.
Communicate effectively and empathetically with patients, carers, and professionals.
Participate in service audits, clinical governance, and continuous quality improvement initiatives.
Receive regular professional and managerial supervision and provide supervision to junior staff as appropriate.
Maintain accurate electronic records and uphold data protection standards.

As a Mental Health Practitioner, you will support adults experiencing a broad range of mental health challenges through comprehensive and therapeutic interventions. You will:
Work collaboratively with individuals to deliver person-centred care based on recovery and resilience.
Utilise interventions such as motivational interviewing, solution-focused approaches, and psychoeducation.
Actively manage risk, including crisis management and safeguarding responsibilities, in complex and sensitive situations.
Be responsible for communicating highly complex information to a wide range of stakeholders and ensuring integrated care planning.
Engage in reflective supervision, team development, and contribute to the leadership of service improvements.
You will play a key role in bridging primary and secondary care and will be expected to work flexibly within the community across multiple sites, supporting a diverse and sometimes high-risk client group. The role also requires proactive involvement in service evaluation, multidisciplinary collaboration, and embracing opportunities for professional growth.
