Job Description

The Wokingham Community Older Adult Mental Health Team is looking for an enthusiastic, caring and highly motivated Mental Health Practitioner with a relevant professional qualification (Social Worker, Mental Health Nurse or Occupational Therapist) to join our friendly and innovative team.



The post holder will be an experienced practitioner with excellent assessment and communication skills, offering support to older people with mental health needs and their carers: providing advice, information and treatment in the service-users own homes and the clinic setting.



Your base would be Wokingham Community Hospital where we have access to free parking facilities, staff room and a friendly and welcoming team. We work flexibly to meet service needs both from the office and remotely in line with service needs.



Wokingham Memory Clinic is proud to be an accredited Memory Clinic by the Royal College of Psychiatrists Memory Services National Accreditation Programme (MSNAP). Combined with being an 'Outstanding', well-led, organisation, and having an 'Outstanding' Older Adults Community Mental Health Team in our latest CQC inspection. The post holder would therefore be expected to work towards helping the service maintain these accolades of high standards.



This post is 22.5 hours a week - core hours 9-5 Monday to Friday

We would consider applicants who wish to work these hours across 4 or 5 days.



The post holder will also offer support to older people with mental health problems and their carers , providing advice, information and treatment in the service-users own homes and the clinic setting . You may be asked to work across the wider service; however, you will primarily be jointly resp onsible for patients on the Memory Clinic caseload . You will work collaboratively wit h the other Memory Clinic practitioners, the wider team and other disciplines from both statutory and voluntary sectors , as well as contributing towards the development of the service.

The post holder will:

undertake comprehensive assessments in a range of settings including the patient's own home



P rovide the appropriate clinical follow up with training and support



I dentify and select from a range of health, social and voluntary agencies which could assist and improve the care of individuals and their families.



B e jointly responsible for the Memory Clinic caseload , meeting the needs of our patients and carers whilst working collaboratively with other Memory Clinic practitioners and wider service.



M onitor and evaluate care plan s , making adjustments as appropriate



H ave excellent problem-solving abilities.

The "must haves" for you to be considered for this role:

Have the relevant professional qualification (Social Worker, Mental Health Nurse, Occupational Therapist,)



You must be enthusiastic, caring and highly motivated with a passion and understanding for working with Older People



You will be an experienced practitioner with the relevant mental health / clinical experience.



Be a car driver with the ability and readiness to travel.
