Job Description

Key Responsibilities:

1. Patient Support and Case Management Provide one-to-one support via face to face or telephone consultation for patients experiencing low to moderate mental health issues such as anxiety, low mood, or stress. Offer psychoeducation such as motivational interviewing and solution focused approaches. Support/signpost patients to access community mental health services, voluntary sector support, and digital mental health tools.



2. Collaborative Working Work closely with the PCN Mental Health Practitioner, GPs, practice staff, and social prescribing link workers. Liaise with IAPT services, and voluntary organisations. Attend PCN meetings.



3. Health Promotion and Education Promote positive mental health and wellbeing within the surgery and wider community. Provide information, advice, and resources related to mental health self-care, stress management, and resilience. 4.



Record-Keeping and Governance Maintain accurate and confidential patient records using SystmOne clinical system. Contribute to data collection and service evaluation. Adhere to safeguarding, confidentiality, and professional standards.
