Job Description
We are seeking a dedicated and compassionate Qualified Social Worker (QSW) to join our Adults Social Care team in Middlesbrough. In this role, you will work closely with vulnerable adults, supporting them in achieving their personal goals and improving their quality of life. Your expertise will be essential in assessing needs, developing care plans, and ensuring the delivery of high-quality services to individuals and their families.
Your key responsibilities will include:
- Conducting thorough assessments of individuals' needs and circumstances.
- Developing and implementing tailored care plans in partnership with clients and their families.
- Providing ongoing support and advocacy to ensure clients receive the appropriate services.
- Collaborating with healthcare professionals, community organizations, and other stakeholders.
- Maintaining accurate records and documentation in line with regulatory standards.
- A recognized social work qualification (Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Experience in adult social care, particularly working with vulnerable adults and their families.
- Strong communication and interpersonal skills, with the ability to build rapport and trust.
- Sound knowledge of relevant legislation and best practices in social work.
