The post is a result of collaborative cancer patients improvement programme aimed to provide people with haematological cancers and myeloid diseases with an excellent experience of care and commitment to support cancer care as the trust top quality priorities.
The post holder will play a key role in developing nursing expertise in Aplastic Anaemia and bone marrow failure diseases to improve experience of a national population.
To work as an expert Aplastic Anaemia/BMF nurse within cancer Multi-Disciplinary Team
To manage a caseload of patients/clients effectively in accordance with agreed National and South East London Cancer Alliance (SELCA) guidance, protocols and pathways of care
To ensure that patients and their families are fully informed and supported by providing clinical, emotional, psychological and social assessment and support in accordance with Personalised Care Standards
To assist in the development of evidence based clinical nursing practice in line with current research and guidelines
To establish clear lines of communication within the specialty team to ensure cohesive multidisciplinary management of patients
To be integral part of established cancer team and specialist MDM, maintaining excellent communication to support coordinated care between sites
To be act as a patient advocate at cancer MDM
To link with healthcare teams in order to increase knowledge and facilitate the care of this group of people in the community
