Neurology Specialist Practitioner- Up to 0.6 B7 North Wiltshire Fixed term maternity coverThe Integrated Community Neuro and Stroke Service is looking for a suitably experienced individual to work as a neurology specialist practitioner with progressive neurological conditions in the North of the county.You will be undertaking comprehensive assessments of the health needs of people diagnosed with progressive neurological conditions, and through proactive case management focus on ways to make demonstrable improvements to those individuals experience of the condition and quality of life.You will have experience of neurological conditions in the community, working with the neuro therapists, knowledge of the specialist services accessible to this client group, and a good understanding of the complexity of presentations. You will be collaborating and liaising closely with a wide range of multidisciplinary professionals in the community, primary and secondary care. You will receive support from the wider team and in particular the other specialist practitioners.We invite applications from nurses or therapists who are interested in this opportunity. The candidates will then be informed of the interview date.Please contact Donna Bayliss on 07825921584 ordonna.bayliss@nhs.netto further discuss this opportunity.Main duties of the jobTo be a highly specialist resource for the Trust, providing evidence based and patient centred care and on-going support for adults with Long Term Neurological Conditions.To use advances clinical skills to plan management programmes that are responsive to the needs of the people with long-tern neurological conditions, their family and carers.To provide support and advice in promoting effective and efficient discharge processes, supporting early discharges from hospital to meet targets of reducing patient length of stay and preventing delayed transfers of care.To act as a specialist resource for knowledge on neurological conditions, providing training and education to develop and support staff of all grades, including patients and carers.To promote and empower self-management through support and education.To maintain Clinical Governance Standards through audit and research.To play a key role in specific symptom management in conjunction with other health care professionals e.g. on-going assessments of treatment for people with deteriorating neurological conditions.To effectively manage own caseload and refer on to other Health Care Professional as appropriate.To effectively communicate with other members of the team to ensure that the service is adequately covered at all times.About usAbout the CompanyWe change lives by transforming health and care.Established in 2006, we are one of the UKs leading independent providers of community health and care services, working with health and care commissioners and communities to transform services with a focus on experience, efficiency and improved outcomes.
