Newly Qualified GP with portfolio role (Primary Care Network)



Salary:

11,000, plus NHS pension and professional indemnity

Working Hours: 4 sessions

Duration: Permanent

Location: Working across Dulwich & Nunhead practices

1. Background



Our South Southwark Primary Care Network (PCN) is made up of all 18 GP Practices in Camberwell, Peckham and Dulwich, approximately 156,000 patients, supported by their GP Federation, Improving Health (IHL). We are a large PCN with 60 staff in post to date and continue to expand over the coming year. We are committed to supporting and developing our staff, knowing this is crucial to our success. We have robust systems in place, including supervision and peer support, and continue to build on this as we expand our PCN team.

Following the introduction of GP roles into the PCN Additional Roles Reimbursement Scheme (ARRS), we are looking for a newly qualified, enthusiastic, forward-thinking GP to join our PCN. This position offers an exciting opportunity to deliver high-quality care through both direct service delivery and contributing to the development of our frailty services. The successful candidate will work as part of a multidisciplinary team to enhance patient care and service delivery.

Job summary



We are seeking an enthusiastic newly qualified GP to join our PCN. This will be a portfolio role which will involve both working within practices and supporting the development of frailty services in the local area.

Practice based work will focus upon



Holistic Assessments: Conduct assessments that consider the physical, emotional, and social aspects of patients' health, aiming to create personalised care plans. Home Visits: Provide comprehensive medical care to patients in their homes, ensuring timely and effective treatment for those unable to visit the surgery. Multi-disciplinary approach: work alongside other healthcare professionals, including nurses, social workers, and allied health professionals, to deliver integrated care.

Frailty Service Development



work with colleagues and inform the design and implement services that address the needs of frail population utilise data and analytics to understand population needs and work with colleagues to develop interventions participate in quality improvement initiatives and contribute to the ongoing development of best practice within surgeries and wider PCN
The successful candidate must be newly qualified (within 2 years) and not previously had a substantive post. You will be provided with the necessary training and development and CPD time within the scope of the role.

Key Responsibilities include



Provide comprehensive and timely care to patients, ensuring appropriate diagnosis, treatment, and follow-up. Undertake home visits for patients in need of medical attention, including those who are housebound or unable to attend surgery who have been referred by GPs. Work closely with practice staff, community health teams, and the wider PCN to ensure continuity of care for patients across services. Ensure accurate and timely documentation of visits, clinical assessments, and treatment plans. Ensure there is appropriate feedback to referring GP/ clinician as required Triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team Manage patients with problems needing referral to secondary care. Ensure service is safe and effective and remains within boundaries of competence, and to acknowledge limitations. Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary Ensure that personal and professional clinical standards are maintained. Communicate at all levels within the team ensuring an effective service is delivered. Maintain accurate, contemporaneous healthcare records appropriate to the consultation. Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high-quality patient care. Awareness of, and compliance with, all relevant practice policies/guidelines e.g. prescribing, confidentiality, safeguarding, data protection, health and safety and QOF standards Achieve and demonstrate agreed standards of personal and professional development to meet the needs of the service. Participate in the audit process, evaluation and implementing plans and practice change to meet patient need.
Duties may vary from time to time, without changing the general character of the post or the level of responsibility.

2. Person Specification



Experience:



Essential:



Medical degree (MBBS or equivalent)

MRCGP or equivalent certification

Within 2 years of completion of CCT

Full GMC registration on GP register

Not previously substantively employed in GP practice

Experience of working in a primary care environment or similar healthcare setting

Experience of dealing with a range of clinical conditions

Experience of providing home visits

Proven ability to evaluate the safety and effectiveness of own clinical practice.

Understanding of evidence-based practice.

Evidence of working autonomously, with some support and supervision

Experience working within a MDT.

Experience using clinical coding.

Desirable



Experience in Triage Knowledge of Quality and Outcomes Framework. Knowledge of public health issues in the local area Awareness of issues within the wider health arena Knowledge of health-promotion strategies

Excellent communication skills (written and oral) including recording of accurate clinical notes.
