To work across health and social care boundaries to obtain further specialist evidence and ensure a co-ordinated approach to care to educate, inform and involve clients, carers and professionals on the National Framework Guidance for Continuing Healthcare (revised 2022) and matters of local service provision. To be actively involved in complaint investigations, contribute to responses to appeals and in monitoring the quality care standards in local nursing home. They will ensure the Head of Continuing Care and the Commissioning Team are kept fully aware of any cause for concern.
