Assistant Practitioner/ Nurse Associate South Somerset Older Persons Mental Health Team.



We are looking for Band 4 Assistant Practitioner/Nursing associates. You will be working as part of the South Somerset Older Persons Mental Health Team and have direct contact with patients, service users or clients in providing high quality and compassionate care for people within the community with mental health needs.



Our Band 4 Practitioners work at a level above that of healthcare support workers and have a more in-depth understanding about factors that influence health and ill-health for people.


Main duties, tasks & skills required

To provide healthcare to patients as delegated by the Registered Nurse/ Mental Health Practitioner. This care will involve physical care and activities which promote health and wellbeing for patients


To work as part of a team, delivering health care that focus on the direct needs of the patient


To work autonomously within the clearly defined boundaries of their role and carry out specific clinical tasks and responsibilities as delegated by the registered nurse / Mental Health Practitioner


To provide planned nursing care and treatment to patients in the community as assessed by the registered nurse / Mental Health Practitioner


To undertake a range of clinical activities for which you have been trained and assessed as competent to undertake, including: SMI checks and Medication reviews.

NMC Nursing Associate Registration (or working towards) An accredited level 5 vocational qualification such as a Foundation Degree or other Level 5 Diploma in Healthcare.
