Job Description

Be responsible for delivering high quality, patient-centred, compassionate care Provide routine care to patients in accordance with clinical based evidence, infection control, health and safety, equality and diversity and safeguarding policies Provide wound care and dressings if required Following training, to provide cervical screening To provide Long Term condition monitoring and on-going management To support and deliver health promotion and well-being programmes including any opportunistic delivery To maintain long term condition registers and support clinics to promote patient safety whilst maximising QOF To support and deliver vaccination programmes for adults and children To maintain accurate clinical records in conjunction with extant legislation To support the clinical team with all safeguarding matters in accordance with local and national policies To support the team in dealing with clinical emergencies To act as a chaperone To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.) To maintain own clinical competencies and be responsible for own continuing professional development and attend appropriate courses and study days arranged by the Practice To support smooth running of the practice
