Job Description

Job Summary:



To provide a high level of care to patients with long term conditions and see and assess patient with acute presentations.

Job Responsibilities:



Management of patients with long term conditions (respiratory, diabetes and cardiac) supported by Senior Nurse Practitioner. Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions.
Provide a holistic approach to wound management and implement wound care in line with current evidence-based guidelines.

Using suitable consultation methods including telephone, video and face to face see and assess patients with acute presentations.

Supporting other members of the team either with clinical advice or training.

Supporting the GPs in the training of medical students.

Diagnose, see and treat patients with acute presentations supported by the GP and wider clinical team.

Assist in the training of final year medical students.

Visit patients at their home (or care home) for acute and long term condition management.

Assist in and perform routine tasks related to patient care. Requesting pathology tests, for example urine culture, swabs Following agreed clinical protocols with referral to nurse practitioner or GPs as appropriate Able to document consultations according to NMC guidelines Ensures awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child/adult abuse and report any safeguarding issues. Able to recognise and manage anaphylaxis according to current UK guidelines Able to perform cardio-pulmonary resuscitation according to current UK guidelines Ensure infection control guides are maintained A duty to advise senior nurses of potential problems or errors within the range of assigned tasks Attend and participate in any practice meetings when required Undertake home visits as required Any other delegated duties appropriate to the post

They may also have access to information relating to the Practice as a business organisation. All such information from any source is to be regarded as strictly confidential Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data

The post-holder will assist in promoting and maintaining their own and others' health, safety and security as defined in the Practice Health & Safety Policy, to include:

Using personal security systems within the workplace according to Practice guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks Making effective use of training to update knowledge and skills Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards Reporting potential risks identified

Infection Control



Use appropriate infection control procedures and maintain work areas in each clinical room so that they are clean, safe and free from hazards reporting of any potential risks identified, including:

hand washing universal hygiene procedures collection and handling of laboratory specimens segregation and disposal of waste materials decontamination of instruments and clinical equipment reporting and treatment of sharps injuries dealing with blood and body fluid spillages assist patients and colleagues in adopting sound infection control measures

Contribution to the Implementation of Services:



The post-holder will:

Apply Practice policies, standards and guidance Discuss with other members of the team how the policies, standards and guidelines will affect own work Participate in audits where appropriate
