Fridays: 08:00 - 20:00
Saturdays: 08:00 - 12:00

Salary:

58,000 per annum (pro rata, based on 40 hours per week)

About the Role


We have an exciting opportunity for an experienced Nurse Practitioner with a background in

respiratory, cardiology, or virtual ward services

to join our innovative Home Hospital service. This service delivers coordinated healthcare to patients in the comfort of their homes, replicating the monitoring and support traditionally provided in hospital wards.
Working remotely, you will assess and support patients with complex conditions -- particularly respiratory and cardiac -- ensuring safe and effective care.

Key Responsibilities

Patient Engagement & Assessment:

Provide clinical assessment, education, and ongoing support to patients within the virtual ward.

Monitoring & Interpretation:

Oversee remote vital sign monitoring (e.g., heart rate, blood pressure, temperature, oxygen saturations) uploaded to our platform 3-4 times daily.

Clinical Decision Making:

Use advanced clinical expertise to interpret trends, identify deterioration, and escalate care appropriately.

Condition-Specific Care:

Manage patients with

respiratory and cardiac conditions

(e.g., asthma, COPD, pneumonia, heart failure) providing guidance on treatment and self-management.

Documentation:

Maintain accurate, timely clinical records.

Collaboration:

Work closely with a multidisciplinary team to deliver coordinated care.

Essential Requirements


Registered with the NMC
MSc in Advanced Practice or RCN Accredited (desirable)
Independent Prescriber
Significant experience in

respiratory and/or cardiology

(e.g., COPD, asthma, pneumonia, heart failure, post-MI)
Previous experience in a

Virtual Ward, Hospital at Home, or acute community care setting


Confident in escalation pathways and managing deteriorating patients remotely
