The following are the core responsibilities of the nurse prescriber. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Conduct clinical assessments and manage patients on anticoagulation therapy (including initiation, monitoring, and dose adjustment). Prescribe and optimize anticoagulation treatment in accordance with local and national guidelines. Review INR results, adjust treatment plans, and ensure timely follow-up.
Deliver patient education and counselling on anticoagulation therapy, adherence, and lifestyle considerations. Maintain accurate clinical documentation and contribute to audits and quality improvement initiatives. Work collaboratively with GPs, pharmacists, and other healthcare professionals to ensure integrated care. Assist in implementing standardised procedures for switching patients from warfarin to DOAC therapy.
