Job Description

This post is open to both Wirral Community Health and Care NHS Foundation Trust and Wirral University Teaching Hospital Staff.



A nursing Assistant Practitioner Role has become available across the Urgent Community Response (UCR) and Frailty Virtual Ward Teams.



This is an exciting opportunity for a candidate to work within an expanding team supporting people being supported to remain in their own home during crisis or periods of illness.



Urgent Community Response and frailty virtual ward sits within the Community Integrated Response Team (CIRT) based at St Catherine's Health Centre and Arrowe Park Hospital, within the Community Response Division.



To support the UCR and frailty ward MDT and be delegated tasks from nursing and community paramedic colleagues. To assess and support with treatment and review of acutely unwell patients in people's own environments, including but not exclusively, medications management, wound and pressure care; phlebotomy, catheter care, clinical observations, supporting health care assistants within the team; being part of in service training programmes and supporting audit.

1.Professional



1.1 Under the direction of the Registered Nurse, provides delegated care to patients in their own home. Plans own day to day work, working mainly alone and unsupervised in a service with fluctuating work loads.



1.2 To work within the community nursing team to contribute to the assessment, planning, implementation and evaluation of appropriate plans of care for patients currently on the caseload



1.3 Liaises with voluntary and statutory agencies Provides and receives routine information requiring tact or information exploring skills within the guidelines of the code of confidentiality and the protection of patient information.



1.4 Keeps accurate records in accordance with the Trust record keeping policy.



1.5 To work within the Trust policies and procedures.



1.6 To undertake a range of clinical interventions, within their own scope of competence that comply with Trust policies / procedures, as delegated by the registered nurse



1.7 Routine monitoring of patients with long term conditions who are medically stable and escalating concerns to lead registered nurse with any concerns of changes to patients health and/or deteriorating medical conditions.



1.8 Recognise, document and report to the Registered Nurse any deterioration in patient's condition.



1.9 Provides appropriate information and educates patients to promote and to enable effective personalised self care.



1.10 Provides support and participates in teaching students, patients and carers



1.11 Liaise/refer and/or signpost to other organisations, including health, social and voluntary.



1.12 Can demonstrate achievement of the core competencies required for this post, in accordance with the mandatory training matrix for the trust and service specific. Attends essential learning and training updates as deemed necessary for the role and the personal development of the practitioner.



1.13 Participates in the orientation of learners and new starters



1.14 Undertakes surveys or audits, when requested.



1.15 Provides/assists in the provision of care for the terminally ill, and is involved in support of their relatives.



1.16 Participates in fitting equipment in patients own homes.



Explains to the patient / family / carer the correct use of the equipment and who to contact if it goes wrong. Reviews whether the equipment is still being used and arrange for equipment to be collected when appropriate. Uses equipment safely in patients own homes, following Health & Safety policies and reporting any problems.

1.17 Ensures nursing equipment is clean, maintained and correctly disposed of when appropriate.



1.18 Reports untoward clinical and non-clinical incidents including near-miss situations to the Registered nurse and complete an incident report via datix .



1.19 Maintains adequate stock levels within base.



1.20 Completes risk assessments of the environment, patient and records and reports any hazards to the Registered nurse in charge .



1.21 Promote the health and wellbeing including personalised care planning of patients/families/carers through health promotion activities.



2. Administrative



2.1 Submit statistical information as required. Completes all other appropriate claim forms / documentation within the given time span and submit to the Registered Nurse in charge



2.2 Attends meetings / courses as required in line with Trust and service specific training matrix.



2.4 Participates in staff performance and development planning process.



2.5 Adhere to Trust policies and procedures with regard to absence from duty.



2.6 Receive, record and respond to information sent either by telephone, fax or computer following Trust guidelines.
