About Us
East Lindsey Primary Care Network (PCN) comprises eight GP practices serving a population of approximately 64,000 patients. Our PCN spans a large and diverse geographical area from North Thoresby to Coningsby and into West Lindsey.
We are a forward-thinking, collaborative network focused on improving patient outcomes and delivering excellent care in the heart of our communities.
About the Role
We are looking for a proactive and compassionate Nursing Associate, registered with the NMC, to join our dedicated team. Youll play a vital role in supporting the delivery of nursing care across our practices, working closely with multi-disciplinary colleagues to support patients with a wide range of health needs.
You will work both within GP practices and in a wider PCN role, with the opportunity to take the lead on areas such as Enhanced Health in Care Homes (EHCH), as set out in the PCN DES contract.
Key Responsibilities
You will:
Provide hands-on nursing care including wound care, immunisations, ECGs, and phlebotomy
Support long-term condition reviews and patient health monitoring
Work collaboratively with GPs, practice nurses, healthcare assistants and other professionals
Contribute to care planning, risk assessments, audits and MDT discussions
Deliver holistic, patient-centred care and act as an advocate for patients and their families
Take responsibility for a designated lead area within the PCN
Ensure safe and effective practice, maintaining accurate records in line with NMC standards
What Were Looking For

A qualified and NMC-registered Nursing Associate
Passionate about providing excellent patient care in a primary care setting
A great communicator and team player
Willing to work flexibly across multiple practices within the PCN
Committed to continuous learning and professional development
Desirable Skills/Experience
Previous experience in general practice or community settings
Experience supporting patients with long-term conditions, mental health needs or complex care
Proficiency in administering vaccinations, medication, and supporting cervical screening programmes (training can be provided)
