Job Description

The post-holder will continue to support the community nursing team but with a wider knowledge and skill set.



You will also support the team in the facilitation of early discharge from hospital where appropriate. The successful applicant will demonstrate a range of clinical skills and competencies along with excellent communication skills providing advice and information to clients and carers with the aim of maintaining them in their own homes enabling maximum independence and optimum quality of life.



The post-holder will primarily support the community nurse team in delivering care to clients with long-term conditions and complex health and social care needs across the community nursing and Intermediate Care Service. You will also support the team in the facilitation of early discharge from hospital where appropriate. The successful applicant will demonstrate a range of clinical skills and competencies along with excellent communication skills providing advice and information to clients and carers with the aim of maintaining them in their own homes enabling maximum independence and optimum quality of life.



We are a large and friendly team where you will have the opportunity to build upon and develop your existing clinical skills. This post requires you to be a team player but confident to work autonomously as required. The locality of Paignton and Brixham covers as far as Kingswear, Stoke Gabriel and Livermead and care delivery will be across this geographical area so the post holder will be required to hold a full driving license and access to a car.



Our service runs from 07:00 - 19:00 7 days a week with various shift patterns and weekend working on a rota basis
