What you will do:

- Support lower-grade Occupational Therapists with complex cases, acting as a mentor under the guidance of the Team Manager.
- Lead care coordination efforts, liaising with various professionals to provide holistic support for service users and their families.
- Accurately maintain comprehensive case management records for monitoring and future planning.
- Deliver efficient and cost-effective health and social care services, working closely with Social Services, Health, the Council, and community organisations.
Requirements:
- You must be registered with the Health and Care Professions Council (HCPC) to apply.
- A valid driver's licence is essential to enable you to visit service users efficiently throughout Islington.
