Job Description

The post holder will be based in the Occupational Therapy service, Mental Health, Coity Clinic, Princess of Wales Hospital.



The post holder will deliver evidence based, recovery focused Occupational Therapy specialist assessment and interventions to service users accessing facilities across unscheduled care (Home Treatment Team, Ward 14 and the Psychiatric Intensive Care Unit).



The post holder will be affiliated with Ward 14 but work within an Occupational Therapy whole team approach to ensure service delivery across unscheduled care. Thus providing continuity of care to service users through their recovery journey and the opportunity to

complete the OT process from admission to discharge.



The post-holder will be supported by a Band 7 Occupational Therapist, work alongside a further band 6 Occupational Therapist and support a rotational Band 5 Occupational Therapist.



The post holder will be responsible for delivering Recovery focussed individual and group Based Occupational Therapy sessions.



All Occupational Therapy assessments and interventions will be evidence based, and evaluated using recognised outcome measures; assessments

and interventions delivered will be measured against a range of performance indicators.



Manage a defined caseload, providing specialist OT assessment and appropriate evidence based intervention, utilising a recovery focussed

model, with individuals referred to the service - promoting independence, recovery and mental well-being within hospital, service users' homes,

and community venues.



Use the Occupational Therapy process autonomously in various settings, including lone working in the community and in service users' homes

across Cwm Taf Morgannwg.



Deliver assessment and intervention inputs using validated and evidence based models of care, assessment tools and outcome measures -such

as the Model of Human Occupation.



Participate in formal meetings, providing expert witness/specialist knowledge of occupational and mental health perspectives.



Work effectively with multidisciplinary (MDT) colleagues to ensure robust therapeutic environs for service users which support autonomy,

maintains function and productivity throughout their inpatient stay.



Contribute effectively to home-leave planning, articulating risk information and opportunities to MDT, and supporting service users to accessing productive and meaningful periods of home leave.

Perform assessments to inform clinical team about future specialist placement needs.

Recommend and liaise with CTP manager to provide appropriate support for the individual liaising with other agencies (i.e. care managers, Platform, Care and Repair, Meals on Wheels, MIND, Primary Care

practitioners, Community Occupational Therapist) to ensure delivery of a seamless service.

Write concise, informative reports detailing assessment and intervention outcomes for service user, referring agencies and other key appropriate services.

Be responsible for maintaining an accurate and comprehensive records of service user interventions in accordance with HPC, local authority standards, Cwm Taf Morgannwg University Health Board

Standards of practice and the Data Protection Act.

Ensure all written and electronic documentation is recorded in a timely manner.

Carry out assessments and interventions within designated target times, requiring excellent time management skills.

Develop specialist skills and expertise within the field of acute assessment, intervention and related outcome measures; Recovery processes; engagement, and risk evaluation and management.

Demonstrate robust understanding of appropriate models and frameworks, including the Model of Human Occupation (MOHO).

Be a point of reference for the team with regards to complex occupational needs within the specialised area.

Be responsible for organising and planning a frequently fluctuating clinical workload. To remain flexible to provide cover at times of staff shortage and allocate work to Occupational Therapists, support staff

and other designated staff as required.

Demonstrate sound judgement regarding achievement of support worker competencies and subsequent delegation of work within a competency based framework.

Contribute to service and strategic developments across Occupational Therapy services.

Contribute in the development of standards and outcome measurement to ensure best practice for service users.

Develop local networks with other teams providing similar services, contributing to the wider professional network.

Undertake reflective practice and to contribute to the debate on a range of clinical issues relating to the

delivery of services for across organisation boundaries.

Participate in and lead tasked working groups in performance, clinical or policy related matters as

identified by line managers.

Participate proactively in Band 6 OT Development forum, feeding back pertinent issues to line

managers as required.

Take a lead role in education and development of competencies of support staff via formal talks, on the

job evaluation, competency based assessments and Agored processes.

Proactively engage with other Occupational Therapists and staff of other professional groups to

progress innovations and improvements in practice.

Maintain awareness of current best practice relevant to field, sharing information with clinical team.

Evaluate clinical outcomes and report outcomes related to service need and areas for development.

Support service evaluation and change processes.

Participate in local, organisational and professional audit and evaluation processes - and support the

implementation of any agreed changes to service provision which arise from same.

Work jointly with the Clinical Lead in identifying methods or changes in practice to remediate

deficiencies.

Work in a manner that ensures compliance with all required policies and procedures.

Provide support and clinical supervision to rotational Band 5 Occupational Therapist

Undertake Performance Development Reviews with identified staff, assuming appraiser role.

Carry out deputising role for line manager.

Use supervision and/or consultation in order to promote personal effectiveness.

Contribute to educating individuals, carers, health professionals on the management of

psychological/mental well-being and occupational needs.

Provide leadership for junior and support staff through supervision, mentoring and modelling best

practice.

Participate in performance appraisal reviews as an appraisee and appraiser.

Take responsibility for maintaining own competency to practice through Continuous Professional

Development (CPD), and maintaining a professional portfolio in accordance with guidelines from the

HPC, which is required for professional registration.

Develop specialist skills and knowledge through an agreed personal development plan.

Supervise students as an accredited practice educator who adheres to the requirements of the defined

standards of practice and supervision.

Provide evidence based training and delivery of competency based practice to the support staff.

Contribute to the delivery of the Level 3 Diploma in Occupational Therapy Support (Agored) delivery

plan.

Deliver and evaluate evidence-based group and individual Occupational Therapy programmes.

Contribute to planning and evaluation of the service through use of audit and research projects.

Develop and maintain a robust awareness of ongoing research relating to your specialist area.

Collaborate with Research Occupational Therapist in post to expand current professional knowledge

fields.
