We are recruiting for Occupational Therapy positions within Intermediate Care Southwark (ICS) - a dynamic, integrated health and social care team.
About the Role:
Carry out comprehensive occupational therapy assessments and develop reablement plans in service users' homes
Deliver personalised, strengths-based interventions that promote independence, wellbeing, and recovery
Collaborate effectively with the multidisciplinary team to support individuals in achieving their goals
Assess and provide appropriate standard aids and specialist equipment, and make referrals for home adaptations where necessary
Complete manual handling assessments, recommend suitable equipment, and develop detailed manual handling plans
Maintain accurate, timely case records and outcome documentation using electronic recording systems
Provide guidance, support, and task delegation to students, junior staff and assistant practitioners
About You:
HCPC-registered and qualified in Occupational Therapy
Experienced in community-based or integrated care settings (desirable)
Committed to person-centred, strengths-based practice
A confident communicator and team player
Organised, proactive, and adaptable in a fast-paced environment
About the Team:
We are recruiting for Occupational Therapy positions within Intermediate Care Southwark (ICS) - a dynamic, integrated health and social care team. Our team includes physiotherapists, social workers, occupational therapists, nurses, pharmacists, occupational therapy assistant practitioners, and rehabilitation support workers. Together, we provide short-term, targeted support to maximise independence and improve quality of life.
Our Rehab and Reablement-focused service supports recovery by optimising individuals' existing skills and resources. We work from a strength-based approach, regularly reviewing care needs and ensuring that any ongoing support is tailored and appropriate.
