Who we are - The Care Team Limited



We are an independent, family led Occupational Therapy service based in Urmston, Greater Manchester, established for over 30 years. Our role is to support the National Health Service by providing assessments and equipment. We are able to offer prompt assessment and equipment provision to aid an individual's safe discharge from hospital or to enable an individual to continue living well in their own home. We often work closely with the therapy teams in the hospitals and community.

We specialise in the assessment and provision of medical grade chairs, ranging from riser recliners to complex posture support chairs and wheelchairs. We see individuals with a wide range of medical needs such as neurological conditions, terminal conditions and frailty.

We will offer any successful candidate the opportunity to develop their physical knowledge and specialist seating skills through training, shadowing and accessing educational courses.

Our team currently consists of two full time and one part time Occupational Therapist. We cover all of Greater Manchester and surrounding areas, conducting assessments in hospitals, care homes and in individual's own homes. We have a professional reputation and work with integrity with the therapy teams in hospitals, step down facilities and the community.

We maintain our reputation by providing effective, patient centred care enabling individuals to continue to live their lives as independently as possible, taking into account individual circumstances.

Overview


We are seeking a dedicated and compassionate Occupational Therapist to join our team. The successful candidate will be responsible for assessing, planning, and recommending equipment to help patients regain their independence and improve their quality of life.

The main role of the therapist is to identify the physical needs of the individual and to have a good understanding of what equipment or adaptations would be beneficial to maintain their independence and quality of life.

This requires a strong understanding of individual care needs, anatomy, and physiology, as well as the ability to work collaboratively with other health professionals and care providers in various settings, including hospitals, care facilities and in individual's homes.

As we are an independent service, the individual may often work autonomously and will take clinical responsibility for their decision making by utilising their clinical reasoning skills, with support from senior care team specialists.

Responsibilities



Conduct comprehensive assessments of patients' physical, emotional, and social needs. Recommending equipment based on assessment findings and patient goals. Deliver, set up and demonstrate various pieces of equipment such as specialist posture chairs and wheelchairs. Implement therapeutic interventions and care plans aimed at improving patients' abilities and independence in daily living activities. To have an understanding of moving and handling techniques and strategies in order to maintain an individual's independence and safety. Collaborate with other healthcare professionals to ensure cohesive patient care. Educate patients and their families about the benefits of equipment and strategies for recovery. Maintain accurate documentation of patient progress through use of our computer system; CRM. Stay updated with the latest evidence-based practices in Occupational Therapy.

Skills



Strong communication skills and effective decision making ability An understanding of physiology, anatomy knowledge, and medical terminology. Experience of working in physical health with an understanding of various medical conditions and how this can impact on an individual's function and day to day life. Experience in prescribing equipment and a knowledge of different types of equipment and the benefits An understanding of posture and how this can be affected through appropriate seating provision An understanding of moving and handling techniques and the development of moving and handling care plans. Experience of case management principles to coordinating patient care effectively. To be able to work autonomously but also joint working with other health professionals and care providers Experience in patient care, ensuring empathy and support throughout the assessment and interventions process. To be responsible for managing own case load, diary and documentation. Must be IT skilled. Confident in delegating work appropriately to other team members

Essential Skills



Degree/Diploma in Occupational Therapy HCPC Registered Experience of working in the community for 2+ years Car driver with a clean licence IT skilled
