If so then we are looking to recruit an enthusiastic and highly motivated individual to join our forward thinking and committed integrated Community Rehabilitation Team at the Midlands Partnership Foundation trust, covering patients residing in Burntwood, Lichfield and Tamworth.



We welcome applications from experienced Band 6 practitioners or aspirational Band 5 Occupational Therapists interested in working in a dynamic, progressive, evidence based team with a passion for supporting good practice throughout the pathway of the neurological patient.



You will manage a varied and complex neurorehabilitation caseload, using specialist assessments and interventions to support individuals with conditions such as stroke, MS, Parkinson's, and acquired brain injuries.



We offer excellent support and development with access to funding for further CPD and opportunities to develop your skills within the four pillars of practice.



The role includes opportunities to supervise staff and students, contribute to service development, and support quality improvement initiatives within a dynamic multidisciplinary team. This post holder would be given opportunities to lead and be involved in various service innovations, fostering an even more integrated approach which affords opportunities to co-design new ways of working and to work in a team which puts people first.



This post holder will require:



A degree in Occupational Therapy and be registered with the HCPC

An up to date portfolio and be committed to CPD

Relevant post graduate training within the speciality of rehabilitation

NHS experience essential

IT skills appropriate for post (All members of the team will receive training on all IT systems)



A car driver is essential for this post due to the nature of our work.

To assess new service user referrals and care/support needs. Assesses service users and formulates, develops, implements and evaluates programmes of treatment /care/support with individuals, families and carers. The specialist care/support plans may include the following:



Promoting independence/self-management and maintaining safety Increased functional ability. Improvement in health and wellbeing including mental health. Interventions in disease/condition management Prevention and reducing of health inequalities. Admission avoidance Early supported discharge * Case management/ treatment /care plans
