Responsibilities

Conduct comprehensive assessments of clients' functional abilities and limitations. Develop and implement personalized care plans to improve clients' quality of life. Identify and recommend appropriate equipment and adaptations to support clients' needs. Perform risk assessments to ensure the safety and well-being of clients. Collaborate with a multidisciplinary team to deliver integrated care solutions.

Person Specification

Qualified with a BSc in Occupational Therapy. Registered with the Health and Care Professions Council (HCPC). Proven experience in care planning and functional assessment. Strong understanding of risk assessment and management. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Commitment to maintaining high standards of professional practice.
