The team...





Our assessment teams use a conversational approach, focusing on an individuals' strengths and the community around them, supporting Gateshead residents to live thriving lives. Teams across the service provide assessment, intervention and support in relation to an individuals' social care needs with a focus on promoting wellbeing and independence, helping individuals take greater control over their lives by championing an early engagement model. Working in this way promotes the individuals access to other agencies and provides them with the tools to be both physically and mentally healthy, reducing their need for long-term provisions and re-enable them where possible.



We are in the process of developing a therapy led prevention model using a reablement approach. As part of the new approach, we are establishing close links with Gateshead Councils reablement service and other community services.



We also have an established service assessing children for equipment and structural alterations and adults for equipment.



About the role....



In this role you will be responsible to the Team Manager to carry out assessments and develop realistic achievable goal plans for individuals that have had a change to functional ability because of a period of ill health. To undertake specialist assessments of the needs of disabled people including children with severe disabilities and life limiting conditions, disabled adults and older persons and their carers in line with the Care Act 2014 and relevant childcare legislation with the aim of improving and maintaining their level of independence in the community by providing Occupational Therapy interventions, including advice, equipment and adaptations delivery.



You should have knowledge or experience of working in a health or social care setting. You will hold a BSC or Masters' degree in Occupational Therapy and be registered with HCPC.



You should have the following essential skills for the role:


IT literacy Good communication skills Good assessment and problem-solving skills Good organisation skills Ability to provide support and mentoring to colleagues The ability to participate within practice education
