Job Description

Active participation in the multidisciplinary team, including joint working, is expected. The role may require working across various wards or units as needed. Additional tasks include collecting and inputting data and statistics, conducting home and access visits, and delivering or fitting equipment at patients' homes before or after discharge. Main Tasks and Responsibilities Clinical Work under the supervision of qualified staff to carry out assigned tasks for individual or groups of patients as part of their treatment and rehabilitation programs.

These tasks may include:

- Contacting relatives and caregivers to gather social and medical history information. - Practicing activities of daily living such as wash and dress practice and kitchen tasks. - Completing referrals for community support and therapy. - Executing treatment plans for patients with physical and cognitive impairments.



Teaching and supervising therapy programs for patients as directed by senior staff. - Documenting in therapy records, communication sheets, medical notes, nursing records, or IT systems as required. - Identifying adverse medical conditions that could impact treatment and responding appropriately. - Collaborating with the team to achieve goals and ensure timely discharge from therapy.
