Older Adults Nurse

to join our expanding team and provide exceptional care to residents in local care homes.


ABOUT THE ROLE



As our Older Adults Nurse, you will play a vital role in delivering high-quality, person-centred care to older adults. Your responsibilities will include:


Care Coordination:

Act as a vital link between care homes and the GP practice, ensuring timely, coordinated care. Attend weekly neighbourhood team meetings and carry out weekly visits to each care home.

Patient Assessment & Management:

Undertake comprehensive health assessments for adults aged 65 and over, support the management of chronic conditions, and work proactively to prevent avoidable hospital admissions.

Non-Medical Prescribing (if qualified):

Prescribe medications and treatments in line with agreed clinical protocols.

Wound Care:

Deliver specialist care for chronic and complex wounds, including Doppler and ABPI assessments.

Immunisations:

Administer vaccinations in accordance with national schedules.

Collaboration:

Work closely with GPs, care home staff, multidisciplinary teams, and families to ensure a holistic approach to patient care.

Additional Clinical Duties:

As required, carry out suture/clip removal, ECGs, venepuncture, and request pathology tests.

The hours of work are 27 hours per week, either on 3 or 4 days. Hours can be discussed at the interview, with some flexibility required to meet patient and service needs.
