Kind Hands Care Services have a full time position available for Care Coordinator. This is primarily office based with requirements to also be out in the field within clients homes to meet with clients and complete supervisions for staff. You will be responsible for on call call outs and emergencies.

Friday, Saturday, Sunday & Monday
You will also be required to take part in the on-call rota 1 x weekday evening
Job Purpose:
To ensure the smooth and efficient running of domiciliary care services by coordinating care packages, scheduling carers, liaising with clients and families, and supporting the care team to deliver high-quality, person-centred care in line with regulatory requirements and company standards.
Key Responsibilities:
Care Coordination & Scheduling
Monitor time-off requests, absences, and availability to ensure adequate coverage
Respond promptly to scheduling changes, including staff sickness and emergencies.
Client Care Management
Assist with the assessment of new clients and creation of person-centered care plans.
Regularly review and update care plans in line with changing needs.
Maintain regular communication with clients and families to ensure satisfaction.
Staff Support and Supervision
Provide guidance and day-to-day support to care workers.
Conduct spot checks, supervisions, and annual appraisals.
Assist with recruitment, onboarding, and training of new staff.
Compliance and Record-Keeping
Ensure all care is delivered in accordance with CQC standards and company policies.
Maintain accurate and up-to-date client and staff records.
Report and record incidents, safeguarding concerns, and complaints appropriately.
Communication and Liaison
Work collaboratively with healthcare professionals, social workers, and local authorities.
Attend care reviews, case conferences, and team meetings as required.
Promote a culture of quality, respect, and continuous improvement.
Person Specification: Essential:
NVQ Level 2 in Health and Social Care(Must be willing to work towards level 3)
Experience in domiciliary or health and social care.
Full UK driving license and access to a vehicle.
Excellent organisational and communication skills.
Ability to work under pressure and handle emergency situations.
Desirable:
NVQ Level 3 (or equivalent) in Health and Social Care.
Knowledge of care planning and scheduling.
Competent IT skills (e.g., rota software, care management systems).
Experience in a supervisory or coordinator role.
Knowledge of CQC regulations and safeguarding procedures.
