An exciting opportunity has arisen to join the ASC Hospital Service as an Operations Manager, overseeing the Hospital Community Discharge Teams and the Continuing Health Care Team. You will play a pivotal role in managing community hospital inpatient assessments and discharges, as well as facilitating smooth transitions through community health pathways.
You will manage a team of Practice Managers and receive regular individualised support and supervision to carry out this role.
Alongside Head of Service, you will help to continue new developments and changes to support the everyday processes within this service.
You will need to be able to demonstrate to have the knowledge and experience to support the following key responsibilities required of this role.
Key responsibilities:
Lead and manage the ASC Continuing Health Care Team and two Community Hospital Discharge Teams
Oversee community hospital inpatient assessments and discharge processes
Manage assessment and discharge through community health pathways
Ensure compliance with statutory duties under the Care Act, including Adult and Carer Assessments, Reviews of Care and Support Plans, and safeguarding adults at risk of abuse or neglect
Supervise and support frontline Practice Managers responsible for adult social care referrals
Chair complex multi-agency meetings, including Adult Safeguarding Enquiries and Best Interest Meetings under the Mental Capacity Act
Authorise complaint and enquiry responses, ensuring investigations meet professional standards and reflect the local authority's position
Lead investigations related to Local Government Social Care Ombudsman enquiries
Collaborate with the Service Management Team to oversee proceedings for Adults subject to Court of Protection applications or Coroners Inquest
We are looking for candidates who:
Can bring significant management and leadership experience
Has relevant Social Work /Professional qualifications
Has the ability to work collaboratively with service management team
Will bring experience of Court of Protection /Coroner's inquest work
Has experience of Hospital /Partnership working
