Job Description

An exciting opportunity has arisen within the Liaison & Diversion Team for the role of Liaison & Diversion Outreach Support, Time, Recovery Worker based in Brighton and covering East and West Sussex.



Liaison & Diversion (L&D) services identify people who have substance misuse, mental health, learning disability or other vulnerabilities when they first come onto contact with the criminal justice system. We look to identify unmet need and vulnerabilities that often lead to or exacerbate offending behaviour, offer onward referral and signposting as appropriate.



We liaise across agencies to support onward planning and intervention to address identified needs and where appropriate support the diversion of individuals away from the criminal justice process into a more appropriate setting.



We offer brief community based intervention to maximise service user access and engagement with appropriate services.



This is an 'all age' service and so you will be required to work with people from the age of 10 years upwards.

The post holder will be expected to travel across the MPFT footprint


The main focus of the outreach service is to support vulnerable adults to engage in the services available in the community.



This may mean supporting people to health appointments, engagement in mental health and drug and alcohol services. Support with accessing online therapeutic interventions that will be available. Providing psycho social approaches, advice and guidance to help support health inequalities and support approaches to help reduce the risks of reoffending.

Main duties and responsibilities:



Working with service users referred to Health & Justice and the individual's carers, helping to promote self-determination, choice and autonomy. To effectively establish a therapeutic relationship with service users and relatives / carer. Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy. Assess patient's individual care needs, agree, and refer on to appropriate agency.

Involve the individual in the planning and prioritisation of their activity where possible. Provide short term intervention to encourage individual to engage with services and attending their initial appointments.

Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence. Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions. Utilise agreed outcome measures to review the effectiveness of process, as part of the Health & Justice Service. Participate in the development of the service to ensure clinical governance processes are actively implemented at team level. Assist in ensuring that the aims and objectives of the service (as set by the Team Leader) are fulfilled and to identify factors which may inhibit these from being achieved. To attend relevant clinical meetings / forums as requested. To plan and manage own delegated workload on a day to day basis. To support with the planning induction of students and new staff. To remain up to date and compliant with mandatory training requirements. Due to the nature of the role the post holder will be required to have a driving licence and access to a car with business insurance as transporting of service users may be required.
