Job Purpose
To be responsible for the provision of support for young people 16 -18-year-old
Main Duties & Responsibilities
1. To assist the young people in their independent living journey.
Provide transport/support to young people as directed in line with support plans.
Conduct welcome and induction for young people moving into the property.
Update of all paperwork, risk assessments, support plans.
Provide ongoing support and monitoring of the young person.
Assist in the management of all health issues, physical, mental and emotional needs.
Preparation of weekly reports to the social workers.
Arrange appointments with various partner agencies.
Maintaining regular contact with young people
Encourage young people to engage with groups in the community.
Provide structure for the young person e.g., register with a college or educational settings.
2. To accurately record, create, maintain and update records in respect of young people.
3. To prepare for and attend meetings as required.
4. To effectively use IT packages.
5. To work effectively and cooperatively across multi-disciplinary teams.
6. To observe and maintain strict confidentiality in respect of the young people and Company.
7. To pro-actively assist in the development of policies and procedures of the Company.
8. To report any housing issues to the maintenance team.
9. To undertake On-Call duties as directed. The out-of-hours on-call coverage currently includes Birmingham and Staffordshire; however, this is subject to change if the business opens new provisions in other areas.
10. Able to show flexibility to cover at different times and locations if required by the Business.
11. Attend mandatory team meetings, training and supervisions on a regular basis as requested by management.
12. Maintain cleanliness of office space and communal areas, ensuring they are well-kept and hygienic for all residents.
13. Perform cleaning duties to prepare rooms for new occupants, including after a young person moves out, ensuring the space is clean, welcoming, and ready for the next resident.

Any such duties should not substantially change the general character of the post. Duties and responsibilities outside the general scope of this post will be mutually agreed with the post holder.

QCF Level 3 in Children and Young People's Workforce or equivalent or working towards it (Essential)
Enhanced DBS record (Essential)
Current full driving license with business Insurance - Cost of business insurance to be covered by the employee (Essential)
Willingness to undertake further training (Essential)
Experience:
In a relative social care environment (Essential)
Experience of working in a supported accommodation (Desirable)
