An exciting opportunity has arisen to be part of our dynamic integrated multi-disciplinary team, SCOPE, working with individuals with a learning disability and/or autism who have complex care needs.



The team supports individuals from the age of 14 years and upwards at different life stages, from transition from school into adulthood, moving to their own accommodation and beyond into older age.



The team is made up of disciplines from both Health and Social Care, such as social workers, Occupational Therapists, Speech and Language Therapists, Community Learning Disability Nurses, Care providers, Family Group decision making, and Psychologists.



The role of the Outreach worker will be intrinsic to this work, to support individuals in their own homes as well as out in the community, inform assessment and support to implement positive behaviour strategies, which will improve the lives of the individuals as well as families, carers and the provider of care.



The Outreach worker will support transitions such as from school to college or their home to an independent living setting, Community access and integration, Volunteering and employment opportunities, emotional support and independent living skills.



This role will include partnership working to develop and facilitate group activities. Outreach workers manage a case load of individuals and work closely with other professional partners such as social workers, occupational therapy, phycology etc.



Outreach Workers provide direct care and support to individuals residing across the whole of Perth and Kinross region.

Skills & Experience Required

We are looking for an individual who has experience of working with vulnerable people in the community, with preferred experience of work those with a learning disability and/or Autism.



An understanding of individuals who are vulnerable or at risk and the care provision they require is preferable.



We are looking an individual who has good communication skills, both verbal and written.



You also need to be able to cope under pressure and respond to the needs of individuals when they are display stress and distress behaviour.



You will have the ability to work flexibility and adapt to the changing demands of the work.



You will be able to work on their own but also work with well within a team.



You will be compassionate in their approach and treat individuals with dignity and respect.



You will be willing to develop their knowledge and skills to meet the needs of those who are the most vulnerable in society to ensure that the have better health and wellbeing outcomes.



To apply for this job, there is a requirement for the successful candidate to be qualified in Social Care (or equivalent) to an SVQ 2 level, or committed to working towards this qualification.



The successful applicant should have an understanding of Adult Support and Protection legislation, and will be subject to a check through the Protecting Vulnerable Groups Scheme.



Experience of working with individuals with a diagnosis of learning disability and/or autism and complex care needs is preferred, but not essential.



It is preferred that the successful applicant has a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.
