Job Description

An exciting opportunity has arisen for Band 6 Paediatric Diabetes Specialist Nurse to join our specialist team, caring for approximately 100 children and young people aged 0-19 years. We are recruiting a permanent position, working 26 hours over 3 - 4 days. We require the successful applicant to generally work Monday - Thursday (3 days one week and 4 days the next) with flexibility to respond to the demands of the service to ensure service cover. The service is based at Dorset County Hospital and works with the population in the locality.



We are looking for a dedicated, enthusiastic and supportive Specialist Nurse to join our forward thinking and innovative Paediatric Diabetes team. The post holder will provide leadership and expert clinical management of diabetes in childhood and adolescence to enable children and young people with diabetes to reach their optimum level of health, independence, social and psychological/emotional well-being, through provision of continuous clinical, social and psychological/emotional support for the patient, their families and carers.



It is possible to offer this post as a band 5 developmental post as Diabetes Staff Nurse for candidates who are interested in becoming a PDSN, with progression to band 6 PDSN following the successful completion of a recognized Management of Childhood Diabetes Course.



The role will involve managing a caseload of patients, providing expert advice to children and young people with diabetes and their families in both the community and hospital setting therefore the ability to drive and use own car for work is essential.



You will be actively involved in quality improvement within the team including contributing to improvements in care in both hospital and community settings through multi-professional, multi-agency working, across primary and secondary health care services and the Paediatric Diabetes Network.



The successful candidate will need to demonstrate leadership qualities, excellent communication skills and be highly motivated to work with the team to continue to develop and enhance our service.

Work with CYPD, their families and carers predominately up to the age of 19 years with type 1, type 2 and other forms of diabetes as part of the Paediatric Diabetes Multi-Disciplinary

Team (MDT).

Be an advocate for CYPD promoting child / young person focussed, family centred care, and act as a resource for others involved with their care

Teach, assess and CYPD, their families and carers (to include education setting staff) to carry out all aspects of diabetes self-care and management, supporting, motivating and

empowering them to enable them to appropriately and effectively manage their condition.

Utilise and demonstrate a good knowledge of safeguarding issues to ensure the child's safety at all time

Work with the Paediatric Diabetes MDT in the delivery of the Paediatric Diabetes Best Practice Tariff

Undertake specialist assessment of CYPD nursing needs; plan, implement and evaluate care using clinical knowledge and skills

Manage a patient caseload as Named Nurse and Key Worker.

Liaise with Health Care Professionals and other professionals (EG education setting staff), who have daily contact with the CYPD in order to provide specialist diabetes care, advice and self-management education in a variety of settings for example; inpatient, outpatient, clinic, home, school, work and college; face to face, by telephone, text Message, email and video conferencing in accordance with Trust guidance.
