An exciting opportunity has arisen for a dynamic and innovative Health Visitor holding a Specialist Community Practitioner qualification (SCPHN) to work as Paediatric Liaison Lead within Gloucestershire Health and Care NHS Foundation Trust, based within the Health Visiting service.

The post holder will lead the Early Years High Impact Area 5:

Improving health literacy, managing minor illnesses and reducing accidents , this includes the ICON programme, the First Dental Steps programme, the Safer Sleep agenda and aligns with the Healthy Child Programme and National Policies aimed at improving health outcomes for children.



The Paediatric Liaison Lead will act as named lead for childhood injury prevention within Gloucestershire Health & Care NHS Foundation Trust (GHC), make links and work with the local authority and multi-agencies on wider determinants of health, such as housing, health and safety. The role supports the development of local pathways aimed at keeping children out of hospital and they have an important role to play in primary and secondary prevention.



Responsibilities extend across the 0-19 public health nursing offer. The Paediatric Liaison Lead will have oversight of the referrals to public health nursing teams from Unscheduled Care to enable data collection and analysis. Local pathways and policies will seek to reduce common unintentional injuries, illnesses and risk of harm.



Unscheduled care referrals to the public health nursing team are received electronically, these are distributed by administrators to the relevant locality team for review and follow-up as necessary.



The post holder will be provided with appropriate IT equipment and access to referral information.

The Paediatric Liaison Lead will provide leadership at a strategic level to contribute to the development and improvement of policies, pathways and strategies to support delivery of high quality, evidence-based, consistent care for children and families for improving health literacy. At a population level, this data will provide a measure of children's development and wellbeing and generate information which can be used to plan services and contribute to the reduction of inequalities in children's outcomes



The post holder will maintain and promote effective communication systems, between GHC Minor Illnesses and Injuries Units (MIIU), Gloucestershire Hospitals NHS Foundation Trust (GHNHSFT) and Primary Care Services to ensure continuity of care for children and families following attendance to unscheduled care. In addition, communication across Child Health Information Services, Children's Social Care, GP's and other services internal and external to GHC will be required.



The Paediatric Liaison Lead will ensure there is an opportunity to achieve long-term health gains for children by sharing information with Health Visiting, School Nursing and Children in Care teams to enable preventative work, targeted support and early intervention to improve the health, well-being and safeguarding of children (including the unborn child).
