Job Description

Working as part of the Aneurin Bevan University Health Board Community Children's Nursing Service and the All Wales Manages Clinical Network (AWMCN), the post holder will lead on the delivery of specialist palliative care and advice.



Adopting a family centred care approach, this role involves supporting families from diagnosis through end-of-life care and bereavement, collaborating with multidisciplinary teams and empowering staff to deliver excellent nursing care delivery in line with evidence based best practice.



The post holder will work with families and members of the multi-disciplinary team, liaising with



inpatient, community and hospice services to co-ordinate and support the delivery of high-quality



holistic care to babies, children and young people who have life limiting or life-threatening conditions, by providing clinical expertise, supporting complex decision making, advanced care planning and



symptom management.

The post holder will expertly assess, monitor and co-ordinate a holistic, individualised, therapeutic plan of care in partnership with the child/young person their families and professionals. The aim of this is to empower resilience and promote quality of life. This will be performed utilising recognised assessment tools as well as expert clinical judgement. Support consultant led paediatric palliative care clinics and run nurse led clinics independently. Be responsible for managing a defined caseload without supervision. Lead and co-ordinate multi-disciplinary assessments and meetings for individual children to ensure continuity of care and support, from all the involved services. The post holder will be required to interpret complex patient condition assessments and multi-faceted situations, comparing a range of options, in order to determine appropriate treatment Provide care in the community, acute and voluntary sector, liaising and co-ordinating across each. Acknowledge and promote best practice, whilst challenging unsafe practice.



Work innovatively, devising novel solutions within own and others' practice to improve quality of life outcomes in an unpredictable environment. Act as a positive role model, playing a prominent role in raising the profile of paediatric palliative care. To work in collaboration with Community Children's Nursing Team to provide support to children, young people and their families known to both services.

We are looking for a motivated, passionate and experienced Paediatric Palliative Care Clinical Specialist Nurse to work within the Community Children's Nursing Service (CCNS) which is part of the Family & Therapies Division with Aneurin Bevan University Health Board.
