We are seeking a highly motivated, enthusiastic and clinically skilled registered nurse with current experience in palliative and acute nursing, to lead and embed an exciting new admission avoidance service working in collaboration with the Acute Hospital at Home and Hospital Palliative Care Teams, enabling palliative patients with acute needs to be cared for at home. The post holder will be expected to exercise a high degree of professional autonomy, make clinical judgements using advanced knowledge and skills to provide the highest standard of holistic patient care, developing and practicing within clinical protocols and guidelines.
The post is for 37.5 hours per week however a minimum of 30hrs/week would be considered for the appropriate candidate.
The post is initially for 12 months and secondments within the Trust will be supported.
To set up, deliver and embed a new service in collaboration with the Acute Hospital at Home (AHAH) Team to avoid hospital admissions of palliative patients with acute needs.
Identify & assess patients with acute palliative care needs in the Emergency Department (ED) & Same Day Emergency Unit (SDEC)
Work collaboratively with the AHAH Team, LED Dr & Pharmacy to establish a medical & nursing care plan to enable the patient to be on the virtual ward at home.
Attend daily virtual ward rounds to review and assess treatment plans.
Daily co-ordination of patient's care needs.
Liaison with multi-professional team - GP, DN's & Weldmar community teams
In collaboration with other key personnel, the post holder will review, develop, and facilitate the AHAH/Palliative Care service, ensuring a cohesive, holistic, and evidence-based approach to management of palliative care patients.
The role will provide specialist nursing leadership in line with local/national guidelines.
