Our Specialist Palliative Care services in Harrow are offering an exciting and innovative opportunity for a community nurse to join our team.

You will work as part of the Specialist Palliative Care Team



You will work in partnership with patients and their families to develop and agree tailored health plans to address their need



You will work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities



You will support learners
