We have an exciting full time opportunity for a Paramedic to join our team.



Responsible for the clinical and proactive management of patients. This role involves diagnosing, treating, and continuously assessing their medical, nursing, and social needs. Responsibilities include prioritising and triaging patients appropriately, making necessary referrals for investigations, and liaising with General Practitioners, Community Matrons, Physiotherapists, Pharmacists, Psychiatrists, and Healthcare Assistants. Additionally, responsible for conducting patient care plan assessments and promoting health and providing health education to patients and healthcare professionals within the local community and care settings.

Job Responsibilities:



Patient Care

: Diagnoses and treats patients effectively of all ages.

Home visits

: Effectively coordinates home visits amongst the team. Preferably can drive or has other means of transport.

Communication

: Ensure effective communication among healthcare staff and act as a link between GPs, community services, and voluntary organizations.

Proactive Engagement

: Supports anticipatory care plans, support adherence to treatments, and promote healthy lifestyles.

Medication Management

: Review medications for effectiveness, provide advice on medication, and ensure compliance.

Clinical Practice

: Maintain safe and effective clinical practice within professional boundaries, manage acute and chronic conditions, and participate in vaccination programs.

Documentation and Workload Management

: Produce accurate records, prioritise and manage workload, and follow guidelines.

Collaboration and Training

: Oversee colleagues, assist in training, contribute to practice strategy, and help achieve quality targets.

Referrals

: Make and accept referrals, utilizing the staff for patient procedures.

Job Type: Full-time

Pay: 22.97-23.65 per hour
