Duties
Provide same-day assessment and triage for patients with undiagnosed or minor illness presentations.
Determine appropriate treatment, self-care advice, or referral to another clinician.
Undertake home visits on behalf of the practice.
Provide urgent assessment and initiate treatment for housebound and vulnerable patients.
Perform and interpret ECGs, venepuncture, and other diagnostic procedures.
Collect and process pathological specimens (e.g., blood samples, swabs).
Prescribe and review medications (if qualified as an independent prescriber).
Support patients in managing both acute and long-term conditions.
Contribute to the monitoring and care of patients with chronic diseases (e.g., diabetes, COPD, hypertension).
Support anticipatory care planning for frail and elderly patients.
Advise patients on lifestyle changes, self-management strategies, and preventive care.
Signpost patients to community, social, and voluntary sector services where appropriate.
Work closely with GPs, nurses, pharmacists, and other practice staff to ensure joined-up care.
Attend and contribute to multi-disciplinary team meetings.
Accept and make referrals as part of integrated patient care.
Participate in audits, service evaluations, and quality improvement initiatives.
Maintain accurate patient records using EMIS.
Ensure compliance with safeguarding, GDPR, and professional codes of conduct.
Support the development of new PCN services and clinics.
Provide teaching, mentoring, and supervision for colleagues and students where appropriate.
Engage in personal CPD and contribute to shared learning across the practice.
