JOB DESCRIPTION
Purpose of the Role
Parenting and behaviour Support Coordinator will work closely with the team to support families identified as requiring support with parenting and behaviour that challenges, attachment, bonding, and supporting with early years development. Families will have been referred through pathways including Health, Flying Start and Families First provisions. Families will usually have children under 5 years of age.
Role Summary:
The role is to provide targeted, time-limited support to families identified as needing support with children's behaviour, attachment, and developmental milestones. Support will be provided directly to parents and children through co-designed plans to achieve measurable outcomes.
Key Responsibilities:
Targeted Family Support
Offer targeted, time-limited support through a co-designed plan for families identified as needing support with children's behaviour, attachment, and developmental milestones.
Provide direct support using parenting strategies and tools to address specific needs and challenges within the home to increase connections and improve family relationships.
Outcome Tracking and Evaluation
Track outcomes for children and parents to monitor progress and measure the impact of interventions.
Regularly assess and document the effectiveness of support provided to families.
Family Engagement and Feedback
Actively engage with families to capture their feedback, ensuring their voices are heard and integrated into the development and improvement of services and policy objectives.
Safeguarding and Compliance
Adhere to all safeguarding practices to ensure the safety and well-being of children and families.
Ensure compliance with all Home-Start Cymru policies, procedures, guidance, and standards.
Collaboration and Partnership
Work closely with your project lead to deliver on identified targets and objectives in a timely and appropriate manner.
Liaise with referring agencies to ensure comprehensive and coordinated support for families.
Centralised Approach to Practice
Maintain a centralized approach to thinking and practice to ensure consistency and effectiveness in service delivery.
Documentation and Record-Keeping
Ensure all relevant documentation regarding the family's journey is accurately recorded and maintained.
Participation in Meetings and Teamwork
Meet regularly with the management team and participate in other relevant internal meetings to discuss progress, challenges, reflective practice and opportunities.
