What Can You Expect?




You will be joining us in the role of

Patient Discharge Co-Ordinator

, located primarily at the

Royal Cornwall Hospital

, with occasional work at our

Redruth base

. In this role, youll play a vital part in ensuring patients experience a smooth and timely discharge from hospital following treatment. Youll work closely with patients, healthcare professionals, and HTG staff to coordinate and deliver efficient transport services, making a real difference to patient care and flow.


This is a

full-time, permanent position (37.5 hours per week, Monday to Friday)

reporting directly to the Customer Relations Manager.

Summary Of Main Duties Include



Coordinate and manage patient discharges through effective communication with healthcare teams and HTG staff. Liaise with hospital discharge points to maintain patient flow throughout the day. Manage expectations within the discharge facility and assist with complex discharges. Schedule patient transport bookings and update the Cleric system. Communicate with HTG transport control centres to ensure timely dispatch of vehicles. Respond to patient queries and provide information on HTG services. Conduct risk assessments and ensure compliance with company policies. Work collaboratively with multiple depots to meet logistical and contractual requirements. This list is not exhaustive.

What Were Looking For:



A good understanding of NHS discharge procedures, patient transport coordination, and effective management of patient flow is preferred. Strong organisational skills and ability to manage multiple priorities under pressure. Excellent communication and relationship-building skills with stakeholders at all levels. High attention to detail and commitment to confidentiality. To hold a

Full UK Manual Driving Licence,

with a willingness to travel if required. Problem-solving skills to handle real-time issues and create practical solutions. Professional, approachable, and resilient in challenging situations. IT literacy and familiarity with scheduling systems.
