We are seeking a compassionate and experienced patient navigator to join our virtual ward team within Acute Medicine.
Acute Medicine has moved to a new model that consists of a Virtual Ward (VW), Medical Same Day Assessment Unit (MSDEC), Rapid Assessment Treatment Zone (RATZ), Assessment Unit (AU), Medical Short Stay Unit (A2) and the Older Persons' Short Stay Unit (A1S). Virtual Ward allows patients to remain at home, the navigator is pivotal in booking appointments at clinic, chasing referrals and results.
The post holder will be expected to work independently to manage the patients on the virtual ward list. The successful post holder will be a kind and caring individual who has exceptional communication needs, and shows a passion and enthusiasm for acute medicine. The Virtual Ward navigator is pivotal in booking appointments at clinic, chasing referrals and results, enabling the patient to stay at home, reducing the need for hospital admission.
